Janssen CarePath Savings Program for PONVORY™
If you are eligible, the Janssen CarePath Savings Program may provide instant savings on your out-of-pocket medication costs for PONVORY™. Depending on your health insurance plan, savings may apply toward co-pay, co-insurance, or deductible. Eligible commercially-insured patients pay $0 per fill for PONVORY™ medication costs, with an $18,000 maximum program benefit per calendar year. Not valid for patients using Medicare, Medicaid, or other government-funded programs to pay for their medications. Terms expire at the end of each calendar year and may change. There is no income requirement.

Mobile Enrollment Available for the Janssen CarePath Savings Program
Use Express Enrollment to check eligibility, enroll, and receive an electronic Savings Program card that can be saved to your digital wallet on your iPhone or Android device. There is also a "Print a Card" feature to provide you with a Savings Program card.
If your pharmacy is unable to process your Savings Program card, you can complete, sign and return the Savings Program rebate form (instructions on form), with required proof of purchase, to receive your rebate check. Click here to get the rebate form.

Janssen CarePath Pretest Rebate Program
Eligible commercial patients pay $0 after rebate for required pretests and/or first dose monitoring when starting treatment with PONVORY™. There is a $1,500 maximum program benefit per lifetime. Not valid for patients using Medicare, Medicaid, or other government-funded programs to pay for their pretests or first dose monitoring. Terms expire at the end of each calendar year and may change. Not valid for residents of MA, MI, MN or RI. There is no income requirement.

To determine if you are eligible for Janssen CarePath Pretest Rebate Program for PONVORY™, call Janssen CarePath at 877-CarePath (877-227-3728) or go to the Express Enrollment site.
Before you submit a rebate request, check the Explanation of Benefits (EOB) from your health insurance provider(s) to confirm that you have an out-of-pocket cost responsibility for your pretests and/or first dose monitoring.
If your out-of-pocket cost responsibility is $0, you should not submit a rebate request. If you have out-of-pocket costs for pretests and/or first dose monitoring, you will need to submit a rebate request using the rebate form. Please follow the instructions on the form and include the required receipts.

Medicare Part D Extra Help — Low-Income Subsidy
Medicare patients who have limited income and resources may qualify for "extra help" to pay for prescription drugs. The Low-Income Subsidy (LIS) program from Medicare provides financial assistance for patients who may otherwise be unable to afford the costs associated with their Medicare Part D plan.
Those who are eligible for LIS may:
- Receive assistance paying their monthly premium
- Have a reduced or no deductible
- Have reduced or no prescription co-insurance and co-payments
- Have no gap in coverage
