Janssen CarePath Savings Program for PONVORY™

Eligible patients using commercial or private insurance can save on out-of-pocket medication costs for PONVORY™. Depending on your health insurance plan, savings may apply toward co-pay, co-insurance or deductible. Eligible patients pay $0 per prescription fill with an $18,000 maximum program benefit per calendar year. Not valid for patients using Medicare, Medicaid, or other government-funded programs to pay for their Janssen medication. Terms expire at the end of each calendar year and may change. There is no income requirement.

Use Express Enrollment to check eligibility, enroll, and receive an electronic Savings Program card that can be saved to your digital wallet on your iPhone or Android device.
You will not have a Janssen CarePath Account and will not be able to view and manage your Savings Program benefits until you create an account at MyJanssenCarePath.com.

Create an online account and enroll at MyJanssenCarePath.com, where you can also manage your Savings Program benefits and receive timely alerts and program updates.

If your pharmacy is unable to process your Savings Program card, you have two options:

  • In your Janssen CarePath Account, you can navigate to the Affordability tab, select Savings Program, click on "Savings Program Request" and follow the instructions to complete the process online.
  • If you don't have a Janssen CarePath Account, you can complete, sign and return the rebate form (instructions on form), with required proof of purchase, to receive your rebate check. Click here to get the rebate form.

Janssen CarePath Pretest Rebate Program

Eligible commercial patients pay $0 after rebate for required pretests and/or first dose monitoring when starting treatment with PONVORY™. There is a $1,500 maximum program benefit per lifetime. Not valid for patients using Medicare, Medicaid, or other government-funded programs to pay for their pretests or first dose monitoring. Terms expire at the end of each calendar year and may change. Not valid for residents of MA, MI, MN or RI. There is no income requirement.

Before you submit a rebate request, check the Explanation of Benefits (EOB) from your health insurance provider(s) to confirm that you have an out-of-pocket cost responsibility for your pretests and/or first dose monitoring.

If your out-of-pocket cost responsibility is $0, you should not submit a rebate request. If you have out-of-pocket costs for pretests and/or first dose monitoring, you will need to submit a rebate request. You have two options:

  • In your , you can Navigate to the Affordability tab, select Pretest Rebate Program, click on Submit Rebate Request, and follow the instructions to complete the process online.
  • If you don't have a Janssen CarePath Account, you can complete, sign and return the rebate form, following the instructions on the form and include the required receipts.

Many states have programs for people with limited income and resources that pay some or all of Medicare's premiums and may pay Medicare deductibles and co-insurance. Find out if your state has a program that can help you.

Medicare Part D Extra Help — Low-Income Subsidy

Medicare patients who have limited income and resources may qualify for "extra help" to pay for prescription drugs. The Low-Income Subsidy (LIS) program from Medicare provides financial assistance for patients who may otherwise be unable to afford the costs associated with their Medicare Part D plan.

Those who are eligible for LIS may:

  • Receive assistance paying their monthly premium
  • Have a reduced or no deductible
  • Have reduced or no prescription co-insurance and co-payments
  • Have no gap in coverage

Medicaid

Medicaid and the Children's Health Insurance Program (CHIP) provide free or low-cost health coverage to millions of Americans, including some low-income people, families and children, pregnant women, the elderly, and people with disabilities.

Some states have expanded their Medicaid programs to cover all people below certain income levels.
